摘 要
  
  目的:口腔鳞状细胞癌(Oral Squamous Cell Carcinoma,OSCC)为口腔颌面部常见恶性肿瘤之一;OSCC 常发生在 40~60 岁的成人,男性多于女性,好发部位以舌、牙龈、颊、腭为多见。OSCC 发展迅速,早期可向区域淋巴结转移,晚期可发生远处转移,严重六合图库患者的生活质量,甚至威胁生命。尽管 OSCC 在治疗方面已取得了很大的进展,但是死亡率仍然居高不下,因此如何实现早期预防、早期诊断及有效治疗,是现代医学界关注的问题之一。近年来的研究热点集中于口腔鳞状细胞癌中异常表达基因的筛选,以助于 OSCC 的早期发现和辅助治疗,提高临床治愈率及生存率。有研究发现 miR21 在头颈部肿瘤组织中表达明显增高,在舌鳞状细胞癌方面研究发现组织中 miR-21 异常表达更为显着。同时我们在前期的实验研究中发现在口腔鳞状细胞癌患者外周血清中存在异常表达的 miRNA.那么 miR-21 在口腔鳞癌组织中的表达是否与肿瘤分化程度有关,目前仍不清楚,因此本研究拟通过比较口腔鳞癌组织和口腔正常黏膜组织中 miR-21 的表达情况并六合开奖直播其与肿瘤分化程度的关系,六合资料口腔鳞状细胞癌发病机制,并为口腔鳞状细胞癌的早期诊断、早期治疗提供实验依据和理论基础。方法:选取河北医科大学第四医院口腔颌面外科 2016 年 9 月至 2017年 9 月初诊为口腔鳞状细胞癌 26 例,行手术切除并病理证实高分化者 10例,中分化者 7 例,低分化者 9 例。切取口腔鳞状细胞癌患者组织和口腔正常黏膜组织(肿瘤边缘 2.0cm 以上)作为实验组和对照组。采用实时荧光定量 PCR 方法分别检测口腔鳞状细胞癌患者肿瘤组织标本及口腔正常黏膜组织标本中 miR-21 的表达水平。结果:

  1. 口腔鳞状细胞癌组织及口腔正常黏膜组织 ODA260/A280 比值均在 1.8-2.0 之间,标本纯度符合要求。
  
  2.口腔鳞状细胞癌组织及口腔正常黏膜组织中 miR-21 的相对表达量分别为 2.401±0.94、0.632±0.15,二者比较差异具有统计学意义(F=99.46,P<0.05)。
  
  3. miR-21 的表达在口腔高、中、低分化鳞状细胞癌组织中相对表达量分别为 2.063±0.81、2.034±0.85、2.739±0.97,低分化鳞状细胞癌组织中较中、高分化鳞状细胞癌组织中增高更为显着 P=0.0242(P<0.05),差异有统计学意义。
  
  结论:miR-21 在口腔鳞状细胞癌组织中表达上调,且其高表达与患者的肿瘤分化程度密切相关,提示 miR-21 可能在口腔鳞状细胞癌的发生、发展中发挥了重要作用。

  ABSTRACT
  
  Objective: Oral Squamous Cell Carcinoma (OSCC) is one of the mostcommon malignant tumors in Oral and maxillofacial region.OSCC usuallyoccurs in adults between 40 and 60 years old, and always more males thanfemales, the most common sites of OSCC are in tongue, gum, cheek andpalate tissues .OSCC developed rapidly, with regional lymph node metastasisin the early stage and distant metastasis in the late stage, which seriouslyaffected the life quality of patients and even threatened life. Although greatprogress has made in treatment, the mortality rate of OSCC is still very high. Therefore, how to achieve early prevention, diagnosis and effective treatmentis one of the issues concerned by modern medicine.In recent years, researchersfocuses on screening of abnormal expression genes in oral squamous cellcarcinoma in order to contribute the early detection and auxiliary treatment ofOSCC and improve the clinical cure rate and survival rate. Some studies havefound that miR21 expression was increased significantly in the head and necktumor tissues, and the abnormal expression of miR-21 was more significant inthe tongue squamous cell carcinoma. Meanwhile, we found abnormalexpression of miRNA in peripheral blood of patients with oral squamous cellcarcinoma in the previous experimental study. So whether miR-21 expressionin oral squamous carcinoma tissues associated with tumor differentiationdegree is still unclear, so the aim of this study is to investigate thepathogenesis of oral squamous cell carcinoma by comparing the miR - 21expression in oral squamous carcinoma tissues and normal oral mucosa tissuesand analyze its relationship with tumor differentiation degree, and in order toprovide experimental basis and theoretical basis for early diagnosis andtreatment of oral squamous cell carcinoma. Methods: From September 2016 to September 2017, 26 cases of oral squamous cell carcinoma were diagnosed in the Department of Oral and Maxillofacial Surgery of the Fourth Hospital of Hebei Medical University.There were 10 cases of highly differentiated oral squamous cell carcinoma, 7 cases of moderately differentiated oral squamous cell carcinoma and 9 cases of poorly differentiated oral squamous cell carcinoma. Oral squamous cell carcinoma (OSCC) patients and normal oral mucosa (tumor margin more than 2.0 cm) were taken as experimental group and control group. Real-time fluorescence quantitative PCR was used to detect the expression of microRNA-21 in oral squamous cell carcinoma patients and normal oral mucosa. Results:
  
  1. The ODA260/A280 ratio of oral squamous cell carcinoma tissues and normal oral mucosa tissues was between 1.8-2.0, and the purity of specimens conformed to the requirements.
  
  2. The relative expression of miR-21 in oral squamous cell carcinoma tissues and normal oral mucosa tissues were 2.401 ± 0.94 and 0.632 ± 0.15, respectively, and the difference was statistically significant (F=99.46, P< 0.05)。
  
  3. The relative expression levels of miR-21 were 2.063 + 0.81, 2.034 +0.85 and 2.739 + 0.97 in the tissues with high, medium and low differentiation of squamous cell carcinoma respectively, and the expression increased in tissues with low differentiation of squamous cell carcinoma was more significant than that in high, medium differentiation of squamous cell carcinoma, and the differences were statistically significant (P=0.0242)。

  Conclusion: Expression of miR-21 was up-regulated in oral squamous cell carcinoma tissues, and its high expression was closely related to the tumor differentiation degree of patients, and results suggested that mir-21 may play an important role in the occurrence and development of oral squamous cell carcinoma.

    口腔鳞状细胞癌(Oral squamous cell carcinoma,OSCC)是头颈部常见恶性肿瘤之一[1].在治疗方面,以手术为主的综合治疗依然是目前最有效的治疗手段,但不同期别 OSCC 患者经诊治后 5 年生存率差异极大[2].恶性肿瘤的发生是一个复杂的生物学过程,正常细胞在多种因素共同作用下,发生基因突变,癌基因激活或者抑癌基因失去活性从而导致细胞的异常分裂、增殖[3].目前研究表明:miRNA 可作为癌基因或抑癌基因,调控鳞癌细胞的增殖、侵袭、转移及凋亡,从而参与口腔鳞癌的发生、发展[4].鉴于此,发现并研究肿瘤发生发展相关的分子机制,探索肿瘤发生发展相关特异性标志物,对于治疗、改善预后显得尤为重要。而阐明 OSCC在分子水平的发生和发展机制可为探索特异性早期诊断分子或辅助治疗提供帮助,这也成为了现今该领域的研究热点。MiRNA 为一类非编码单链 RNA 分子,长度约为 20~25 nt,调控约30%人全部基因,通过与靶基因 3‘端非编码区(3'-untran slated regions,3'-UTR)完全或者不完全配对,导致目的基因沉默或阻断目的基因的翻译过程,以发挥转录后调控因子的作用[5].miRNA 在肿瘤发生和发展的过程中,在细胞分化、增殖、凋亡、代谢、侵袭性及耐药性等多种生物学行为中发挥着癌或抑癌基因的作用[5-6].有研究表明:miRNA 可用于肿瘤治疗[5,7],以抑制癌基因或促进抑癌基因的表达来发挥治疗作用。这为 miRNA应用于 OSCC 的早期诊断和辅助治疗提供了可能性。近年来,miRNA 的异常表达与包括 OSCC 在内多种病变之间的密切联系被不断揭示出来,如食管癌、结肠癌及胰腺癌等病变中均发现 miRNAs 的异常表达。Verbin RS 等[8]报道:miR-21 在胃癌[8-9]、胰腺癌[10]及卵巢癌[11]等多种肿瘤中均表达上调,可能发挥着癌基因的作用。本文通过检测 miR-21 在口腔鳞状细胞癌肿瘤组织标本及口腔正常黏膜组织标本中的表达水平,进一步六合开奖直播 miR-21 与口腔鳞状细胞癌肿瘤分化程度的关系,六合资料 miR-21在 OSCC 发生和发展中发挥的作用,评价其在临床诊疗方面的价值,为提高 OSCC 的早期诊断率以及完善患者的综合治疗方案提供新思路。
